documents required :
1. manually signed commercial invoice in six folds certifying that the goods shipped as per contract/order no.005/006
2. packing list in six folds
3. full set clean shipped on board bill of lading plus three non negotiable, made out to the order of pt. bank mandiri(persero)marked 'freight prepaid' and notifying applicant with full name and address
4. beneficiary certificate supported by courier receipt stating that one set copy of shipping documents have been sent directly to applicant after shipment effected
5. insurance certificate in triplicate to the order of opening bank in 110 pct of invoice value, in all risk condition covered by shipper, and claims (if any) are payable in indonesia
